The checklist is as follows:[\/vc_column_text][vc_column_text]<\/p>\n<h3>Physical Harm<\/h3>\n<p>\u2610\u00a0Develop robust protocols for triaging, quarantining, and distancing to minimize exposure.<br \/>\n\u2610\u00a0Minimize contact with patients by using telehealth when possible and cutting back on schedules.<br \/>\n\u2610\u00a0Provide sufficient access to personal protective gear.<br \/>\n\u2610\u00a0Ensure the availability of and easy access to hand hygiene products.<br \/>\n\u2610\u00a0Communicate and enforce guidelines for reducing risk of transmission.<br \/>\n\u2610\u00a0Communicate and enforce return-to-work guidelines for infected workers.[\/vc_column_text][vc_column_text]<\/p>\n<h3>Emotional Harm<\/h3>\n<p>\u2610 Communicate frequently and empathically to build trust.<br \/>\n\u2610 Consistently employ universal leadership skills such as communication, awareness, honesty, and transparency.<br \/>\n\u2610 Organize peer support groups.<br \/>\n\u2610 Promote and support health and wellness practices around nutrition, sleep, exercise, and social connections.<br \/>\n\u2610 Offer and suggest anxiety-reduction strategies and opportunities such as mindfulness exercises and buddy systems.<br \/>\n\u2610 Provide professional counseling and mental health services through employee assistance programs.<br \/>\n\u2610 Establish and communicate clear policies around time off, sick time, and pay.<br \/>\n\u2610 Design solutions for providing childcare coverage.<br \/>\n\u2610 Develop and communicate clear guidelines to minimize family exposures.<br \/>\n\u2610 Offer support for families of caregivers.[\/vc_column_text][vc_column_text]The <a href=\"https:\/\/www.hospitalmedicine.org\/globalassets\/practice-management\/practice-management-pdf\/pm-20-0011-strategies-for-hospitalist-wellbeing-during-covid19-m.pdf?v=jHaUUdv3v_0\" target=\"_blank\" rel=\"noopener noreferrer\">Society of Hospital Medicine<\/a> (SHM)<a href=\"#notes\"><sup>3<\/sup><\/a> held a webinar on April 22,2020, and recommended the following strategies:[\/vc_column_text][vc_column_text]<\/p>\n<ol>\n<li>Transparent and frequent communication from leadership<\/li>\n<li>Increase collaboration within\/between groups to reduce patient loads and increase time for self-care<\/li>\n<li>Build partnerships between the group\/institution and the local community to address personal stressors<\/li>\n<li>Provide resources to help with stress management; increase access to optional counseling sessions<\/li>\n<li>Create opportunities to recognize and share positive news and success<\/li>\n<\/ol>\n<p>[\/vc_column_text][vc_column_text]The <a href=\"https:\/\/www.ama-assn.org\/delivering-care\/public-health\/caring-our-caregivers-during-covid-19\">American Medical Association<\/a> (AMA),<a href=\"#notes\"><sup>2<\/sup><\/a> on June 17, 2020, updated their recommendations to include the following strategies for health care leadership to support staff:[\/vc_column_text][vc_column_text]<\/p>\n<ol>\n<li>Assess physician stress and identify specific drivers<\/li>\n<li>Build a resilient organization<\/li>\n<li>Workload redistribution<\/li>\n<li>Institutional policies \u2013 changes in paid time off and sick days, ensure no expense for COVID-related illness<\/li>\n<li>Meals \u2013 providing access to meal delivery<\/li>\n<li>Providing childcare and pet care<\/li>\n<li>Adequate access to PPE<\/li>\n<li>Paying attention to emotional and mental well-being<\/li>\n<li>Providing adequate social support<\/li>\n<\/ol>\n<p>[\/vc_column_text][vc_column_text]All three organizations recognize the importance of addressing barriers to both the physical and emotional wellness of their providers during the pandemic. The Press-Ganey checklist provides a concrete list of goals that it recommends institutions achieve whereas the SHM and AMA provide broader guidelines for categories of initiatives that should be addressed, and examples of good practices.<\/p>\n<p>While the Press-Ganey checklist provided an initial framework to address hospitalist well-being needs, it did not adequately address how to best support hospitalist workload through adequate staffing, nor how to appropriately assess stressors at individual sites&#8211;both of which we thought were fundamental to evaluate. For this reason, we developed a questionnaire based on the Press-Ganey checklist with modifications recommended by the AMA and conducted a directed assessment of how HOMERuN institutions addressed provider wellness during the pandemic.
